Personally I like discountASP.NET - http://www.discountasp.net/features.aspx
I found them to have the best price for the most features when I was looking. Both US & Euro datacentres.
They meet or exceed all of the items on your shopping list except for mySQL.

Uk based, seem very responsive to any issues, initial setup for shared server seems fine (love the ease of using fantastico, should be a must on all hosts). I don't think the version of sql server is more recent than 2005 though - having said that, I'm sure if you really needed 2008 they'd arrange something.
EDIT If you need more than a few domains, try the reseller option. Doesn't mean you have to sell on web space; works out at just a couple of quid more per month

I've always found Crystaltech to be very good and have used them for a number of years. On the rare occasion i've had an issue its been sorted super-quick. US based though, but its never bothered me.

Windows Web Hosting
After my current host (http://www.hostdone.com) nearly cost me some work due to them being stupidly crap/slow decided to it's time to move. Anyone know any good, reasonably priced hosts with the following:
Disk space (1GB+)
Bandwidth (10GB+)
Multiple Domain hosting under same plan (either unlimited or up to about 20)
.NET 3.5 (and lower version)
MSSQL (2005 and/or 2008)
Silverlight
PHP 5.2
MySQL
Basically it's for domain parking/Playtest area and occasional client demo's, so does not have to be A++ hosting but it does at least need to useable.
Not fussed if it is US or UK basedTags: None
