You could go ultra simple and design an Excel spreadsheet on Google Docs if you can't find anything better. Not terribly secure, but it would tick the simple to use and access box.

Free online defect tracking system

Anyone know of a bug/enhancement tracker that a client of plan b can use to raise defects and enhancement requests and where I can quote for each item, prioritise, close and report on the items raised?

More features the better but needs to be really easy to use.

Usual sort of thing.

Looking for something already hosted if possible rather than installing it on my servers.
