Weird Android phone speaker / mic problems

I disabled mic permission for Google. No dice.
Did factory reset. No dice.
Installed phone doctor plus. Says hardware is fine.

Handset speaker has now stopped working. But if I do phone doctor plus test, I can not just here the test noises but it also allows any open app e.g. radio player to put sound through speaker.

Any ideas from anyone vaguely sober?
