Ordering from USA: Royal Mail handling fee

Has anybody ever ordered goods from the USA for a value slightly exceeding £15 and not paid the Royal Mail handling fee of £8? I would like to order a supplement from the USA via International Airmail and the value in pounds would be around £16-17. In theory, because it's over the threshold of £15 I should pay VAT + handling fee.
On many forums I've read that this rarely happens.
Any personal experiences? Insights? Thanks
