    Where I work currently it is banned. You are very lucky!

    Make sure everything is very positive. If you have to put -ve stuff then say something like "works too long hours" or something like that.

    Yes, been there myself. What you do in the hours that they pay you for is up to them. Bend over, carry on invoicing.

    Give good, positive feedback to everyone, yet ensure this looks like it is very well thought through.

    Even if working with the most incompetent oik on the planet who has shagged your missus, ensure this is worded in a very positive manner.

    You want all permies to love you, and also want the person you are giving feedback to to think this is well considered.
