For some contracts I have my own one but if they provide one then I use that and fill it out how I like to. The current one has stuff like what tasks you've been doing. I just put all types of crap. When I worked at Porsche, the agency said I had to use their online SAP based system for timesheet and stuff like that but I had to refuse. The online system only worked with Internet Explorer and Porsche only allowed Netscape! Same with the Americans I work for, everything is either Notes or MS based and the expenses and timesheet system is Great Plains which also only works with IE. I just send them an invoice now, don't even bother with a timesheet.
