My accountant made a mistake last year (1st time in over 13 years with them) that lead to a £100 fine. They re-imbursed me.

Seems the 'get rich for nothing and sue 'em' culture is alive and well with some contractors!
I couldn't give two fornicators! Yes, really!

Just to be absolutely clear, I don't want to sue them and I don't want to get rich. I do NOT want to profit from this. I just don't want to pay for a service I didn't get, and it didn't seem right that they weren't acknowledging that. When I've made a mistake before in a contract I've corrected it in my own time to ensure I deliver what I said I would deliver - and got more work because of how I handled it. I wanted to find if there's a way to raise my problem with the accountant as an issue in order to improve the service for others rather than just shrug and leave them to it let them do it to someone else, though agree that's the easy way out. Perhaps I'm an idealist/optimist hoping that people can learn from others mistakes. I get that it's a trivial issue, low value, unimportant etc, but I just wanted to see what others had experienced so thanks for all your thoughts.

Good on you for not naming and shaming them, very amicable.

For what its worth, id expect a goodwill gesture or some serious grovelling as I totally understand its not just the fine, but the stress and time taken to sort it. Also I get nervous with anything I feel may draw attention from Hector

FWIW, I think you're being perfectly reasonable; no problem asking the question. I do think that the answer is to accept what you've been offered in this case though, and look for another accountant. It's important that you're confident about the work they're doing, and this would shake my confidence too.

P.S. Ignore trolls. They have nothing better to do but make crass comments.
